Oregon Student Health Survey

During the week of March 29th - April 2nd our 6th, 8th and 11th graders will be given the opportunity to participate in a survey regarding their health. This information will be valuable when planning for student support in the upcoming school year. The Oregon Department of Education has worked closely with the Oregon Health Authority to incorporate questions in the Student Health Survey to identify how COVID-19 has affected students and their households. These survey questions are available for community review in the attachments below. This survey is not mandatory and students will be given the option of opting not to participate.
